A favicon should be 16 and 32 px for browser tabs, 48 px for some screens and search results, 180 px for iPhones, and 192 and 512 px for Android and installed web apps. Start from a square image of at least 512 px and the pack covers every size.

A new favicon often doesn't show because browsers cache favicons for a long time. Reload with the cache cleared or open the site in a private window, and check that /favicon.ico opens on its own. Search results can take weeks to update.
